In ‘Mayor of Kingstown’ Season 3 Episode 9, Iris visits Mike and informs him that Milo is still alive.
Mike, however, is skeptical and worried about Iris’ mental state. Feeling distressed and ill, he suggests that she stay there for the night to ensure his safety.
Later, while talking to Constantine, Iris comments that she and Milo are two sides of the same coin, which angers Constantine. He is visibly disappointed by the comparison and insists that Iris leave Kingstown.
His insistence on leaving town reflects his fear that staying in Kingstown could put his life in grave danger. It seems Constantine knows Milo is alive and he can kill Iris.
In the end, Roman (Mark Ivanir) goes to collect the papers for Iris’ new identity, hoping for her safe escape from Kingstown. However, he unexpectedly encounters Milo, who was presumed dead but is very much alive.
The surprise meeting quickly turns violent as Milo, intent on asserting his power and foiling any plans to protect Iris, shoots Roman in the leg. It serves as a chilling message about Milo’s ruthless determination to eliminate anyone who stands in his way.

Looks like Milo is back to get revenge on both Mike and Constantine. His return signals a dangerous escalation in their ongoing conflict. Milo’s primary goal seems to be to cause maximum pain and disruption, and he knows that targeting Iris will achieve that.
Iris holds significant importance to both Mike and Constantine. By threatening or trying to kill Iris, Milo can strike the heart of his enemies.
Milo won’t let Iris leave Kingstown safely and may even try to kill Constantine, who killed Milo’s lover Tatiana (Gratiella Brancusi). Milo’s vendetta against Konstantin for Tatiana’s death means that he is probably plotting to eliminate him as well.

If Constantine discovers that Milo wants to target Iris, it could start a violent showdown, which could cause chaos in Kingstown. Constantine’s protective instincts towards Iris will push him into a terrifying confrontation.
As their battle intensifies, the fragile balance of power in Kingstown will be disrupted. Constantine may try to kill Milo before he harms Iris because he cares deeply for her. However, if Constantine discovers that Iris is with Mike, it could shatter his trust and cause a rift between him and Iris.
Milo, always looking for an advantage, can use this information to further weaken Constantine. Constantine may believe that Mike is using Iris as leverage or that Iris chose Mike over him.

The revelation would cause considerable emotional turmoil for Constantine. As Constantine struggles with his feelings of betrayal and renewed threats from Milo, Iris’s safety becomes more uncertain. He could have become a pawn in Milo’s game, using both Mike and Constantine.
Considering how much Bunny Washington (Toby Bamtefa) hates Constantine, the other gang members may also choose Milo over Constantine. Milo, as a master manipulator, will exploit the tension between Bunny and Constantine. By positioning himself as a more favorable ally, Milo could easily gain the support of those who saw Constantine as a common enemy.
